1.4.2 Print Report screen is empty; no controls except for Back button.

When I select Time and then Print Report on the General Timeslots banner  I get a blank screen.  This is after upgrading to 1.4.2

Is it just me?

It was just me.

I somehow fat fingered an edit that I put it to show 5 group by categories.

Funny how a little change like:

  <?php for ($i = 1; $i <= 5; $i++){ //mwg 3->5

can go so wrong...
