user list display error - can't select to subscribe

After upgrade (I can't remember which) the listing of users displays very strangely (avatars overlap text, text overlaps itself) and we now can't click any text or avator to 'subscribe' or 'invite' people to an object.

Current version stated is 1.5.3.

We've also lost avatars at some upgrade - and now are unable to update a users' avatar getting a 'Failed to Edit Avatar" error.

Here's the debug log we get is after trying to subscribe someone (clicking on their name or image) and then trying to apdate an avatar:

We've tried upgrading a couple of times,  but this didn't help.

Have you tried upgrading an installation of the same version installed in your server, to the latest first, before doing so on your main? Because you mention a 1.5.3 one, which is like 1.5-2 years old..
Remember backing up your data once you are going to upgrade the main installation.
